ABSTRACT

Nowadays, with the outbreak of COVID-19, the prevention and treatment of COVID-19 has gradually become the focus of social disease prevention, and most patients are also more concerned about the symptoms. COVID-19 has symptoms similar to the common cold, and it cannot be diagnosed based on the symptoms shown by the patient, so it is necessary to observe medical images of the lungs to finally determine whether they are COVID-19 positive. As the number of patients with symptoms similar to pneumonia increases, more and more medical images of the lungs need to be generated. At the same time, the number of physicians at this stage is far from meeting the needs of patients, resulting in patients unable to detect and understand their own conditions in time. In this regard, we have performed image augmentation, data cleaning, and designed a deep learning classification network based on the data set of COVID-19 lung medical images. accurate classification judgment. The network can achieve 95.76% classification accuracy for this task through a new fine-tuning method and hyperparameter tuning we designed, which has higher accuracy and less training time than the classic convolutional neural network model. © 2023 SPIE.

ABSTRACT

This study examines clinical outcomes in patients with cytomegalovirus (CMV) and SARS-CoV-2 coinfection. Between June and November 2020, previously immunocompetent patients with SARS-CoV-2 and CMV coinfection were identified at Houston Methodist Hospital as part of routine clinical correlation by a molecular pathologist. SARS-CoV-2 nasopharyngeal specimens were analyzed by real time reverse-transcriptase polymerase chain reaction (RT-PCR). All CMV tests were performed on plasma or bronchoalveolar lavage (BAL) specimens and analyzed by competitive polymerase chain reaction. 65 previously immunocompetent patients with CMV and SARS-CoV-2 coinfection were identified. Patient demographics include 41 male patients (63%) and 24 female patients (37%) ranging in age from 34 to 86 years (mean: 66.04, median 68). Documented pre-existing conditions include 27 patients with hypertension 41.5%), 19 patients with diabetes mellitus (29.2%), 9 patients with coronary artery disease (13.8%), and 3 patients with asthma (4.6%). Eight patients (12.3%) had no documented pre-existing conditions. The plasma CMV viral load ranged from <300 to 21,566 IU/mL. The CMV PCR results from bronchoalveolar lavage and bronchial wash specimens ranged from <300 to 59,127 IU/mL. CMV PCR was initially negative in 10 patients then positive on serial testing. 60 patients were critically ill requiring ventilator support (92.3%). 47 patients (72.3%) expired, 7 patients (10.8%) were transferred to a long term acute care facility, 3 patients (4.6%) were discharged to a rehabilitation facility, 3 patients (4.6%) were discharged home, and 1 patient (1.5%) remained in-patient at the time of analysis. The prevalence of CMV seropositivity and medical comorbidities increases with age. Reactivation of latent CMV is a known occurrence in critically ill patients that is associated with poor outcomes. The majority of the patients in our cohort were 50 years old, and all were severely to critically ill with a mortality rate of 72.3% These findings suggest CMV portends a worse prognosis in patients with COVID-19. These findings also demonstrate the importance of clinical correlation in molecular testing.

ABSTRACT

The impact of air pollutants on human health is a major issue that developing nations are facing in last decade. Effect of particulate matter especially PM2.5 and PM10 has been severely affecting mortality and morbidity in Rajasthan state as per global lead medical journal Lancet recent publication. Twelve air pollutant-monitoring sta tions in Jaipur city are not enough to predict the exact concentration of air pollutants in each of the 91 wards of Jaipur. In absence of accurate concentration at micro level, it becomes a major challenge for urban planners to remedial strategies. In this paper through GIS spatial distribution, a comparative study of particulate matter at each of the 91 wards during pre-lockdown for the year 2019 and post-lockdown 2020 was done. Results for 2020 showed slightly less pollution;similarly, air purity index, an indicator for healthier environment, was determined for each ward. © 2023 Scientific Publishers. All rights reserved.

ABSTRACT

Case history: We present the case of a 31-year-old Hispanic male with history of recurrent bronchiectasis, invasive aspergillosis, and severe persistent asthma, who is now status post lung transplant for end-stage lung disease. He initially presented at 7 years of age with diarrhea, failure to thrive, and nearly absent immunoglobulin levels (IgG < 33 mg/dL, IgA < 7 mg/dL, IgM = 11 mg/dL, IgE = 4 IU/dL) necessitating IVIG treatment. Small intestinal biopsy showed villous atrophy consistent with autoimmune enteropathy. Sweat chloride was reported as indeterminate (44 me/dL). Initial WBC, platelet, and T- and NK-cell counts were within normal range, and B-cell count and percentage were borderline low. Most recently, he was found to have increased immature B-cell count (CD21low), decreased memory B-cells, and poor pneumococcal vaccine antibody response. Patient has been hospitalized numerous times with increasingly severe bronchiectasis, pneumonitis, and COVID-19 infections twice despite vaccination, leading to respiratory failure and lung transplantation. Family history is negative for immune deficiency and lung diseases. Discussion(s): Of these 3 VUSs (see the table), the one in IRF2BP2 has the most pathogenic potential due to its autosomal dominant inheritance, its location in a conserved domain (Ring), and previous case reports of pathogenic variants at the same or adjacent alleles 1-3. Baxter et al reported a de novo truncating mutation in IRF2BP2 at codon 536 (c.1606CinsTTT), which is similar to our patient's mutation. This patient was noted to have an IPEX-like presentation, with chronic diarrhea, hypogammaglobulinemia, and recurrent infections. Variant Functional Prediction Score for our variant predicts a potentially high damage effect. There are 2 other case reports of heterozygous mutations in loci adjacent to this allele;one (c.1652G>A)2 with a similar clinical phenotype to our patient and the other (C.625-665 del)3 with primarily inflammatory features and few infections. Impact: This case highlights a variant in IRF2BP2 associated with severe hypogammaglobulinemia, recurrent pulmonary infections, and autoimmune enteropathy. ABSTRACT

The successful elimination of the SARS-Cov2 virus has evaded the society and medical fraternity to date. Months have passed but the virus is still very much present amongst us though its severity and contagiousness have decreased. The pandemic which was first detected in Wuhan, China in late 2019 has had colossal ramifications for the societal, financial and physical well-being of humankind. Timely detection and isolation of infected persons is the only way to contain this contagion. One of the biggest hurdles in accurately detecting Covid-19 is its similarities to other thoracic ailments such as Lung cancer, bacterial and viral Pneumonia, tuberculosis and others. Differential observation is challenging due to identical radioscopic discoveries such as GGOs, crazy paving structures and their combinations. Thorax imaging such as X-rays(CXR) have proven to be an efficient and economical diagnostics for detecting Covid-19 Pneumonia. The proposed work aims at utilising three CNN models namely Inception-V3, DenseNet169 and VGG16 along with feature concatenation and Ensemble technique to correctly predict Covid-19 Pneumonia from Chest X-rays of patients. The Covid-19 Radiography dataset, having a total of 4839 CXR images, has been employed to evaluate the proposed model and accuracy, precision, recall and F1-Score of 97.74%, 97.78%, 97.73% and 97.75% has been obtained. The proposed system can assist medical professionals in detecting Covid-19 from a host of other pulmonary diseases with a high probability. © 2023 IEEE.

ABSTRACT

Acute severe symptoms and long-term sequelae caused by the novel coronavirus disease (COVID-19) are still major concerns for public health. In particular, it is an emerging need to prevent the overburn of health workers as well as the collapse of health care systems. To reach this purpose, it should be necessary to evaluate a standardized pre-hospital management for COVID-19 patients, but data about it are lacking. Thus, the aim of the present chapter is to analyze the in-hospital gesture hypothesizing its reproducibility at bedside.Meta-analyses and randomized clinical trials assessed were focused on the following topics: (1) early diagnosis through viral demonstration and serological testing;(2) home setting evaluation;(3) standardized multidimensional assessment of COVID-19 patients, including an early identification of specific clinical symptoms as well as a prognostic stratification through laboratory biomarkers and portable imaging techniques;(4) safe and easily administrable drugs, considering both new medications and repurposed molecules;(5) protocols regarding bedside oxygen therapy, prone positioning, and pulmonary rehabilitation.To date, several procedures for the in-hospital management of COVID-19 patients could be easily and safely applied in the outpatients' care. The institution of dedicated international open-access data banks could be useful to realize standardized pre-hospital protocols, and the implementation of remote approaches could provide the possibility to guarantee a continuous follow-up for these patients. ABSTRACT

Introduction: COVID-19 related encephalitis has been reported in pediatric patients;however, there are no reports in patients with inborn errors of immunity (IEI). Activated PI3K Delta Syndrome (APDS) is a disease of immune dysregulation with immunodeficiency, autoimmunity, and abnormal lymphoproliferation resulting from autosomal dominant gain-offunction variants in PIK3CD or PIK3R1 genes. We investigate a family with APDS, one mother and three children, one of whom developed COVID-19 related encephalitis. Method(s): Patients were consented to an IRB-approved protocol at our institution. Medical records and detailed immunophenotyping were reviewed. Family members were sequenced for IEI with a targeted gene panel. Result(s): The index case is a 10-year-old female with a known pathogenic variant in PIK3CD (c.3061 G > A, p.Glu1021Lys), who contracted SARS-COV-2 despite one COVID-19 vaccination in the series. Her disease course included COVID-related encephalitis with cerebellitis and compression of the pons, resulting in lasting truncal ataxia and cerebellar mutism. At that time, the patient was not on immunoglobulin replacement therapy (IgRT), but was receiving Sirolimus. Besides the index case, 3 family members (2 brothers, 1 mother) also share the same PIK3CD variant with variable clinical and immunological phenotypes. All children exhibited high transitional B-cells, consistent with developmental block to follicular B cell stage. Increased non-class switched IgM+ memory B cells and skewing towards CD21lo B cell subset, which is considered autoreactive-like, was observed in all patients. Of note, the patient had low plasmablasts, but normal immunoglobulins. Of her family members, only one was receiving both sirolimus and IgRT. Conclusion(s): We describe a rare case of COVID-19-related encephalitis in a patient with inborn error of immunity while not on IgRT. This may indicate infection susceptibility because of a lack of sufficient immunity to SARS-CoV-2, unlike the rest of her family with the same PIK3CD variant.Copyright © 2023 Elsevier Inc.

ABSTRACT

The purpose of this research is to study the effects of neoclassical trade liberalization policies enacted in India in 1991 to determine the effect on levels of poverty and income inequality. This research predicts that poverty and economic inequality will be reduced due to implementation of economic liberalization policies. The research uses empirical data from the National Sample Survey Organization (NSSO), in India and develops a regression model to determine the effects of economic liberalization on income inequality and absolute poverty. The results of the regression model suggest that income inequality and poverty decreased during the year liberalization policies were enacted, but is not statistically proven with enough confidence that liberalization is strongly correlated with a reduction in inequality and poverty. There is a weak statistical correlation that suggests inequality increased in the Indian urban sector, and decreased in the rural sector due to liberalization. In conjunction with a literature review where more robust data and econometric models are applied, the empirical analysis by complimented with the fact that in general income inequality decreased due to economic liberalization policies alone, holding all exogenous factors that affect income inequality constant. The literature review also confirms that poverty levels decreased with economic liberalization, holding all other exogenous factors that affect poverty constant. The implication of this research is that liberalization polices have been successful for overall development in India, and suggests that implementation of liberalization policies may be desirable in nations under similar circumstances as India in the era before its liberalization. © The Editor(s) (if applicable) and The Author(s), under exclusive license to Springer Nature Switzerland AG 2022.

ABSTRACT

This research hypothesizes that greater availability of healthcare services, and greater choice in healthcare facilities results in better health when controlling for a variety of socio-economic factors within the Canadian context. This research will model access to healthcare services using density of general and specialist physicians relative to population size, and the geographic density of healthcare facilities. Choice in healthcare is modeled by the number of healthcare facilities in each health region, when normalized by the population in that health region. Various health outcomes will be used as benchmarks to test this hypothesis, including self-reported general health, self-reported mental health, influenza immunization rates, body mass index (BMI), and incidence of diabetes, cardiovascular disease and hypertension. From the empirical results, choice in the healthcare system does not have an impact on the selected health outcomes. Increased availability of healthcare generally improves health outcomes, but this is dependent on the health outcome in question, and the provincial region being analyzed. © The Editor(s) (if applicable) and The Author(s), under exclusive license to Springer Nature Switzerland AG 2022.

Touch-based fingerprints are widely used in today's world;even with all the success, the touch-based nature of these is a threat, especially in this COVID-19 period. A solution to the same is the introduction of Touchless Fingerprint Technology. The workflow of a touchless system varies vastly from its touch-based counterpart in terms of acquisition, pre-processing, image enhancement, and fingerprint verification. One significant difference is the methods used to segment desired fingerprint regions. This literature focuses on pixel-level classification or semantic segmentation using U-Net, a key yet challenging task. A plethora of semantic segmentation methods have been applied in this field. In this literature, a spectrum of efforts in the field of semantic segmentation using U-Net is investigated along with the components that are integral while training and testing a model, like optimizers, loss functions, and metrics used for evaluation and enumeration of results obtained. © 2022 IEEE.

ABSTRACT

Background: COVID-19 pandemic still pose a substantial threat worldwide despite increasing vaccine availability. Patients with haematological malignancies have been shown to have increased risk of contracting COVID-19 and are more susceptible to develop severe illness from SARS-CoV- 2 infection. The immune response to vaccines is impaired in patients with haematological malignancy due to underlying disease or antineoplastic therapies. The monoclonal-antibody combination, Evusheld is composed of tixagevimab and cilgavimab, two neutralising antibodies against SARS-CoV- 2. It has been shown to be safe and have efficacy for the prevention of COVID-19. Our aim of study is to describe the incidence and outcome of breakthrough COVID-19 infection among patients who received Evusheld in our centre and analyse the factors that possibly increase the risk of breakthrough infection. Material(s) and Method(s): A retrospective review of all adult patients with haematological malignancy who received tixagevimab/ cilgavimab 150/150 mg injection in Hospital Pulau Pinang from 1 July 2022 to 31 August 2022 with a follow-up period to 30 November 2022 was conducted. Demographic data, clinical characteristics and outcome will be retrieved from patient's medical records. Data were analysed using Statistical Package for Social Sciences software (version 21.0). Result(s): A total of 96 patients (50 males and 46 females) received tixagevimab/cilgavimab injection during the study period with a median age of 61 years (range 19-82). Majority of them were diagnosed with multiple myeloma (42.7%), followed by lymphoma (33.3%) and leukaemia (24%). One third of them had history of therapy with monoclonal antibody and 20% had haematopoietic stem cell transplantation. No major adverse effects of tixagevimab/cilgavimab injection were noted among the study population. Of the 12 patients (12.5%) who had COVID-19 infection, all of them had mild infection;three were asymptomatic and six patients received Paxlovid antiviral therapy. The median time from tixagevimab/cilgavimab to the onset of COVID-19 infection was 35 days (range 5-97 days). The mean age of patients with breakthrough COVID-19 infection were older compared to those without breakthrough infection but was not statistically significant. The incidence of breakthrough COVID-19 infection was not affected by type of haematological malignancy, history of monoclonal antibody therapy or COVID-19 vaccination. Discussion and Conclusion(s): Our findings showed that tixagevimab/cilgavimab was safe and effective in preventing COVID-19- related morbidity and mortality among patients with haematological malignancy during the study period. However, the limitation is the lack of access to whole genome sequencing for detection of resistant variants for breakthrough infections.

ABSTRACT

The purpose of this study was to examine the impact of first-year seminar (FYS) course modality on first-semester GPA and second-semester retention of community college students' system-wide. For many student's community colleges serve as an affordable option for higher education. The first-year seminar course connects students to the college to create better academic outcomes. With the increase in technology the FYS course is available in three different course modalities;face-to-face, hybrid, and online. Understanding which course modality has the largest academic outcomes of FYS course will present administrators of the benefits of one modality over others. The study investigated academic outcomes of the FYS course by examining course modality at Lone Star Community College (LSC). Findings revealed no statistical differences between full-time enrolled students in any of the course modalities. Part-time students in the face-to-face FYS had higher first-semester GPA's than those enrolled in the other modalities. Results revealed students in the online FYS course modality were less likely to be retained the second semester. This research focused on examining the motivational factors that determine the behaviour of consumers in pre and post pandemic times in South Africa by conducting a secondary literature study in the selected universe, with the aim of exploring the key factors in the changes of consumption in men and women, due to the situation of isolation caused by COVID 19. The economic factor referring to the consumption of basic necessities in consumer behaviour is a benchmark for marketing sanitary. Because cleaning, sanitizing and medicinal products have become part of the basic (emerging) consumption of men and women in South Africa. Likewise, the psychological factor, from an exercise and health perspective, is another important factor in times of pandemic. The present study conducted in two phases, pre and post COVID-19 in South Africa. During those period consumers used to buy in a hybrid mode of shopping. Twelve variables (items) are used for the scale development of buying behaviour and taken from the literature and further improved through EFA (Exploratory Factor Analysis) in the study. The questionnaire was developed using the five-point Likert scale. The survey tool consisted of 30 items. There are 258 respondents were replied through online questionnaire. Two set of questions were asked, and responses were analysed.

ABSTRACT

This qualitative case study explores the development of pre-service science teachers' (PSTs) understanding of Socio-Scientific Issues (SSI) instruction. The study utilized a conceptual framework of SSI-based instruction and a three-phase approach involving co-designing solutions, co-designing curriculum materials, and co-teaching classes. Primary data sources included PSTs' interviews and reflective journals, while artifacts, field notes, and curriculum materials served as secondary data sources. Thematic analysis was used to analyze the data of six PSTs in a teacher preparation program. We found that the PSTs' understanding of SSI instruction was enhanced in 12 features of three core aspects: design elements, learner experiences, and teacher attributes. "Engaging in higher-order practices" was the most prominent feature, observed across all three phases. The three-phase approach played a crucial role in promoting PSTs' understanding of SSI instruction, with each phase guiding their understanding in distinct ways. Particularly, the co-designing solutions phase facilitated the development of PSTs' "awareness of the social considerations associated with the issue". While the co-designing curriculum materials phase was effective in promoting their understanding of "scaffolding for practice: providing scaffolding for higher-order practices. "The co-teaching classes phase played a crucial role in facilitating their understanding of "willingness to position oneself as a knowledge contributor rather than the sole authority".

Introduction: STAT1 gain-of-function (GOF) disease is associated with chronic mucocutaneous candidiasis (CMC) and a broad spectrum of infectious, inflammatory, and vascular manifestations. The Janus Kinase inhibitor ruxolitinib has been used successfully for CMC and autoimmune phenomena. We describe a case of warm autoimmune hemolytic anemia (WAIHA) in a patient with STAT1 GOF disease after initiating ruxolitinib. Case report: A 36-year-old man with STAT1 c.850G>A (p.Glu284Lys) mutation presented with CMC as well as recurrent viral and bacterial infections, lymphadenopathy, enteritis, nodular regenerative hyperplasia (NRH) and splenomegaly. Immune workup confirmed a combined immunodeficiency with hypogammaglobulinemia and T-cell lymphopenia. Ruxolitinib was initiated at 5 mg twice daily (due to pre-existing thrombocytopenia) with up titration over 3 months to 20 mg twice daily. He improved with weight gain, increased energy, resolution of chronic anemia, and improved lymphadenopathy and splenomegaly on imaging. Serum CXCL9 only minimally decreased from 4660 pg/ml to 3990 pg/ml. Soon after reaching ruxolitinib 20 mg twice daily, he developed JC viremia, prompting dose reduction to 15 mg BID. Within two weeks, he developed a non-COVID upper respiratory tract infection followed by fatigue, shortness of breath with ambulation, and dark urine. Emergency evaluation revealed warm antibody positive hemolytic anemia with a hemoglobin of 5 g/dL, and worsened thrombocytopenia. He was treated with blood transfusions, pulse steroids, and high-dose IVIG with stabilization but continued hemolysis. Due to the JC viremia, there was concern to give rituximab with increased PML risk. Bone marrow showed trilineage hematopoiesis, a mild increase in megakaryocytes and RBC precursors, and a loss of B-cell progenitors with retention of mature B cells. His B and T lymphocyte numbers had increased since prior to ruxolitinib, with a predominance of Tfh1-cells (58.7% of total Tfh-cells). He was started on sirolimus with a slow taper of prednisone with continued stable hemoglobin and platelets, and resolution of hemolysis after 3 months. Conclusion(s): To our knowledge, this is the first case of a STAT1 GOF patient developing WAIHA while receiving ruxolitinib therapy. Treatment choices were complicated by the risks of PML. Sirolimus combined with ruxolitinib allowed wean of corticosteroid and subsequent resolution of hemolysis.Copyright © 2023 Elsevier Inc.

ABSTRACT

Since the beginning of the global Covid-19 pandemic, text media materials are full of the word "vax”, and after the appearance of vaccines against the coronavirus and the start of the vaccination campaign around the world, "anti-vax” has also been added. In the article, it is singled out the linguistic means of updating the evaluation in the headlines and leads of the text media of Ukraine in the materials dedicated to opponents of vaccination against Covid-19, and the possibility of its automatic recognition with the help of machine methods is also considered. It was found that among the language means of expressing assessment, colloquial vocabulary (jargonisms and slang) and phraseology come to the fore. © 2023 Copyright for this paper by its authors.
